Social prescribing focuses on people using their strengths, relationships, and community resources to improve their health and well-being.

How it works
Participants meet with a Community Health Worker over six weeks to build the three building blocks of health:

Connection - Feeling supported and part of community
Confidence - Believing in your ability to make positive changes
Control - Feeling empowered to shape your life and health
Through strengths-based coaching, participants identify their strengths, set small meaningful goals, connect with local activities, and build supportive relationships.
Launched with Orchid Health Wade Creek Clinic, the program connects patients to support beyond medical care.
